In Sunday a week ago's Arizona Republic, there was an article about how several FD's in the Phoenix area want to get away from contracts with Rural-Metro (PMT/Southwest) and run their own rescue/medic units.

Several cities have applied for or received the appropriate certificate from the state health department to do so.

Peoria is one of them.

So look for PMT/Southwest, and the upstart AMR, to be limited to inter-hospital runs, rather than having 9-1-1 contracts with several cities in the next few years.

It's not limited to the Phoenix area either. Northwest Fire/Rescue down in Tucson got themselves a shiny new Certificate of Need from the state and now handles all the ALS transports in their district. Northwest still calls in a Southwest Ambulance branded taxicab for the really BLS calls. This is the model Tucson Fire has used for several decades. Yuma Fire started up this model a few years ago too.

One wonders whether the trend will hit the handful of smaller ambulance companies in the state too. Lakeside Fire District (which has a CON) merged with Show Low and Linden Fire Districts (which did not have a CON) to form the Timber Mesa Fire District up there. One would think they'd want to migrate to a consistent transport model for the entire district, which could spell trouble for the for-profit Show Low EMS (run by Arrowhead Mobile Healthcare). Bryan Savage is the chief up there now, and he's a former Northwest Fire guy.

And apparantly, AMR is intending on buying out Rural/Metro. So instead of AMR being a competitor to R/M (PMT/SWA), they'll be one big monopoly.

Just clarification, AMR is not buying Rural Metro. Envision Healthcare, which ows AMR, purchased Rural Metro. AMR and Rural Metro will be merging technology and resources, but names will remain. With that being said, the new "business" model for AMR and Rural Metro is to get away from providing all levels of 911 for a subsidy, and move towards joint ventures, where the FD and AMR/RMA share transport services. Many contracts have been renegotiated this way (RMA in Orlando for example) and it saves the taxpayers and the city/county money because AMR/RMA now only make money from billing patients, and the city/county now has the ability to double resources at no cost to them.
