Default Looking for older UHF MED Channels listings

I know in 2005, Florida updated the UHF MED Channels listings across the state and there was no deadline on when the counties were to switch over. In my area, they still use the older listing. For example, I heard the following “Gulf Flight 3 to Baptist ER on Med-7”. Baptist uses 463.1625. The old listing shows 463.1625 as MED-7. On the new 2005 list it shows it as MED-72. Does anyone have an older listing so I can update my database to reflect what my counties are using? As you can see, my channels area kind of mixed up between old and new.
Thanks
Dan M

Below is what I'm woking on in my area.

Okaloosa County
Frequency* Tone* Description*
462.9500 156.7 MED-9 EMS Dispatch
463.0250 156.7 MED-2 EMS to FWB Medical Center & Twin Cities Hospital
463.0500 156.7 MED-3 EMS to Eglin AFB Regional Hospital
463.1750 167.9 MED-8 EMS to North Okaloosa Med Center

Santa Rosa County
Frequency* Tone* Description
453.9500 218.1 N/A EMS Dispatch (Rual Metro)
463.0000 136.5 MED-1 County Medical Coordination
463.0625 136.5 MED-3 EMS to Santa Rosa Medical Center
463.0875 136.5 MED-4 EMS to Gulf Breeze Hospital
463.0375 136.5 MED-2 EMS to Jay Hospital

Escambia County
Frequency* Tone* Description
462.9750 110.9 MED-10 EMS Dispatch
463.0125 110.9 MED-1 EMS to Navy Hospital (Pensacola)
463.0625 110.9 MED-3 EMS to Sacred Heart Hospital (Pensacola)
463.1375 110.9 MED-62 EMS to West Florida Hospital (Pensacola)
463.1625 110.9 MED-7 EMS to Baptist Hospital (Pensacola)

Walton County
Frequency* Tone* Description
463.0750 167.9 MED-4 EMS Dispatch
463.1500 167.9 MED-7 EMS to Walton Hospital

Just some gee-wiz info if you monitor this area. EMS does not transport to just there assign counties. Example, Santa Rosa transports to both Escambia and Okaloosa also. Walton EMS transports to both Walton and Okaloosa. This is because Okaloosa and Escambia have larger health care facilities. In order to be able to follow the action, it’s best to have them all programmed into your scanner.
