Same as you can without the extreme...
You can only assign an alert tone in a favorite list - you do not need extreme to do it.

I would add that, you can add the desired alert tones in a Favorites list via Sentinel, regardless of whether the Extreme Upgrade was purchased, or not.
However, with the Upgrade, you can make that change on the scanner itself, without using Sentinel, but it still has to be done in a Favorites list, not the main database.
