I'd say:

FL Dept of Transportation
- location #5 is about 2 mi away from "90th Ave"
- Fort Lauderdale, FL (BROWARD)

Plus, FLDOT uses a lot of 141.3 PLs and a few 143 DPLs.

Google reveals a helluva lot of 90th avenues in the US though. But I'll hedge my bet on this one.
