Renewal Form

This is what I get when I try to print the renewal form to mail with my payment. Is is mandatory to send this with my payment?

I've fixed this for you.

Yes, you need to fill out the forum and mail it in with your payment. I can't tell you how many times I get a payment in the mail that is just a check.... with no information whatsoever.
