If one wants to cancel their premium membership does the fee get pro-rated and balance refunded? Or is it just a loss?

Apparently, at least some refund-requests are unclear/nonsensical, and might get posted here for everyone's education/awareness.
In other threads, you will find a rare refund being sent for very specific reasons, but in general, the "no-refund" policy seems to solidly stand since it was part of the required-reading prior to becoming a member here.
If you feel you must pursue the matter, then it probably would be best to handle the matter privately by emailing the site owner a polite request with full explanation, and letting the matter stay private, without ranting all over these forums if your request is not granted.
