An XTL5000 with the flashcode _APPL5_ is that even possible?
Anyone have any idea what's up with this?

This is in fact a valid tag. I have a couple of these in our shop that came from an agency in California, I believe it is an application flashcode for a radio that has been sent back to the depot at one time for repair or a special application testing radio. It is uncommon, but they do exist. I have only seen these come from the LA County and San Diego County areas. I have a couple in UHF R2 band and a few in 700/800mhz. All of the ones I have are exactly the same flashcode in them regardless of band 500008-000000-1 and they all have DVP-XL Encryption in them.

Custom firmware options are common in any radio based gear these days.

Some of the RFID transponders I worked on, licensed 800 & 900MHz ISM stuff had maybe 20 options. A lot depended on on regulatory jurisdiction, which varies worldwide. Sometimes, a configuration option wasn't quite possible for a particular use case and a custom firmware (actual custom code - not config options, ala "codeplugs" etc). would be needed. Those units would be the ones with the weird, one-off, serial numbers.

No, custom firmware is not common in radio's..it is uncommon, especially in public safety radio's. This flash option _APPL5_ has nothing to do with firmware.

You're probably be right in respect to LMR - I forgot that my personal experiences are in the ISM markets where it's mostly data transmission and the need for integration with a customer's data systems (computers and/or network) - and needs vary. Wildly. We try to design a system that'll cover most bases - but there's always one that out there, and if we can make a buck on it, we'll do it.
