Can anyone provide the correct frequency tables for the state ALMR P25 system for a Uniden? Can't find my cheat sheet for converting the RS/Whistler one that's shown in the database entry.

No, that table only works for Radio Shack and Whistler. Uniden uses a different format, which should also be listed, but isn't.
How old is this Uniden you’re using? All the scanners from at least the past 8 years, automatically populate the tables once they’ve tuned the control channel for a few seconds. You shouldn’t need to manually do it.
Anyway, here’s what the Unidens automatically populate nowadays:
0: 851.00625/6.2500
1:762.00625/6.2500
(And then 2: through 9: are all the same)
2:136.00000/12.500
