I've been listening to Austin Fire/EMS and noticed a few things. One is that I don't hear any Medical. Another thing I've noticed is the dispatch format ( unit number, call type, in "AFD box [whatever], location, repeat), but what is does the "AFD box" mean?

It depends on how their box cards are set up but, box cards show two things, 1)what apparatus are first due for a specific area and the list of due apparatus to an area if first due is busy. 2)where hydrants are in that box. But like I said, it's agency specific and I'd bet that its option 1 more than anything.

I've been away for awhile but if i recall correctly, For example Box 214 1st number stands for the Battalion and the other 2 stand for the map page for that Battalion District,

I hear Medical related calls dispatched all day long 154.175 154.205. When they are on a medical call they respond usually on the medical talkgroups such Medcom North, Medcom South, Medcom west, Medcom East. North and South is usually for city calls, and Easy and West for county calls.

The city is divided into Boxes. Station 28 by me would respond to calls in Box 2801, Box 2802, etc. here is a station Map:

So box has nothing to do with the lockboxes outside of all businesses that the FD has a key to? I know different FD's have the keys to different lockboxes (which hold a key to the business).

Correct. the Box Alarm box number corresponds with the Austin Fire Station Number. It has nothing to do with the lockboxes on a business.
