Re-enable a deprovisioned feed?

My feed was off-line for a few weeks and was deprovisioned. What e-mail address do I contact to get it re-enabled now that I've solved the scanner <--> server communications problem?

You will need to re-apply for the feed. We are no longer able to re-activate deprovison feeds.

Make sure to include all info like freq or talkgroup info.
