stereo y-cable that splits left and right to mono connectors

Do anybody know what Kind of stereo y-cable that splits left and right to mono connectors need and where to purchase that would Split two Scanners Audio to the computer to the Broadcastfity stream. i'm thinking about hook two scanners up and split the audio from the two scanners and stream it on my Broadcastfiy stream like have the 800 mhz Scanner stream on the left and the VHF Scanner stream on the Right. Thanks

Also, you should add an isolation transformer between each scanner and the adapter. I’ve used these and they work well:
