We have a case here where a common county name is repeated within the same state.

We have a disambiguation category in the wiki reserved for just this purpose, but I dislike using it, because it's rather sloppy. Montgomery county as an entity exists in several states.
How about making an unique name for the phase 2 system entry, such as Montgomery County, PA (P25)? There is a precedent for the comma and state abbreviation - see the Maryland entry for the same county, which is written precisely in this manner.
We can then temporarily move the associated wiki article so that I can build a proper wiki page to house the data already there.
