When I submit information to the Database, for some unknown reason it will duplicate it. Does anyone else have this issue. I only hit the submit button once.

FWIW,
It can happen when you refresh the submission page, or perhaps navigate back then forward again in the browser. I think this happened to me before, the DB Admin simply ignored the duplication..
To prevent that I close the submission window, and return to review submissions via my RR user page. (not refresh, or go back)
