Subscription renewal

My Subscription expires shortly and I would like to send a check for the year. It says there is a form I have to fill out, is this correct? When I click on the PDF icon, nothing happens. Your assitance would be appreciated.

Just noticed that 90 day renewal is not an option anymore. I'm I doing something wrong or now I can only get 180 or 360 day subscription?

EDIT: That's via paypal, but pdf mail in form still has other options.
