What is an Air Plane Crash Code?
Do you mean Alter I, II, or III?
Do you mean Alter I, II, or III?
Varies from area to area - but goes something like this:
•ALERT 1 = Minor malfunctions that Don’t constitute threat to aircraft or passengers
•ALERT 2 = Situations if not contained WILL endanger aircraft and passengers
•ALERT 3 = Imminent or actual crash of an aircraft on or near the Airport

The Phoenix (AZ) FD Regional Dispatch Center uses these exact codes. I believe the Mesa (AZ) FD Regional Dispatch Center also uses them.

Those are standard FAA codes, used at airports throughout the US.

There are only three ALERT codes.
There some level indicators covering the number of people on the aircraft and there are some modifiers that indicate the type of crash or emergency. They are all easily found in an internet search for the FAA documents.
Had a crash about a mile from the end of the runway a few years ago and it was dispatched as an Alert V.

Alert 5 must be an internal code used by your local emergency services. As said, ATC Alert Codes are 1, 2, and 3.

Was it actually Alert 5 or maybe Level 5
Some Airports also use a Level, which indicate the # of Souls on Board
