WELD COUNTY - A two-car crash killed a little girl Friday after she was ejected from a sports utility vehicle near Fort Lupton.

The Colorado State Patrol says the 5-year-old girl who died was unrestrained at the time of the crash.

Two other children were in the SUV along with the female driver and an adult passenger. They were taken to an area hospital but the extent of their injuries was not known. Additionally, two passengers and the driver of the pickup truck were taken to the hospital. Their conditions are also not known.

Subject: On June 06, 2008 at approximately 1:02 PM a five (5) year old girl was killed in a head-on crash on U.S. 85 south of the interchange of Hwy y 52.

A 2003 GMC Envoy driven by Esperanza Garza age 39 of Fort Lupton, CO lost control while traveling southbound U.S. 85. The vehicle ran off the left side of the road, traveled through the center median into the northbound lanes where it collided with a 2005 Ford F-250 pickup truck driven by Bradly Courtnage age 43 of Thornton, CO.

The five year old girl was a passenger in the GMC, she was not restrained in a child passenger seat. The girl was ejected from the GMC and suffered fatal injuries. She was pronounced dead at the scene.

Garza (mother of the 5 yr. old) suffered minor injuries and was treated and released at the scene. A 16 year old female passenger Tiara Jacovetta of Fort Lupton suffered minor injuries and was treated at North Colorado Medical Center in Greeley. Jacovetta was wearing her seat belt. A 2 year old male (son of Garza) suffered minor injuries and was treated at NCMC.
The 2 yr. old boy was properly restrained in the vehicle. Isreal Magdaleno age 19 and son of Garza suffered moderate injuries and was treated at NCMC.
Isreal was not using a seat belt.

Courtnage, driver of the Ford truck suffered minor injuries and was treated at Good Samaritan hospital. Courtnage was wearing his seat belt. A passenger, 33 yr. old Teresa Anderson of Loveland, CO suffered minor injuries and was treated at Good Samaritan. Anderson was wearing her seat belt. A second passenger Caleb Courtnage age 10 suffered minor injuries and was treated at Good Samaritan. Caleb was wearing his seat belt.

State Patrol investigators have determined that Garza was attempting a lane change from the right lane to the left lane when she lost control, traveled into the center median, rolled one full time and landed in the northbound lanes where it collided with the Ford truck.

Alcohol/drugs are not suspected. Any and all charges that Garza face are pending. Northbound Hwy 85 was closed for 2.5 hours during the investigation and clean-up. One southbound lane remained open throughout.

This is the official report, understand that some of the earlier verbal information was incorrect.
