Translated for the Pro-96, that should be Base=470.0000, Offset=12288, Step=6.25. You'll need to program all the control & alternate control channels but programming voice-only channels into the Pro-96 is a waste. Based on the channel information listed by ScanXO this one configuration will suffice, if they ever add frequencies below 470 or above 500 then a multi-table would have to be programmed.

It appears to this amateur that they have incorrect input offsets for UHF, that they should be 5, 3, and 3 for those sub-bands. I wonder if it is deliberate, as I have seen that once before on a control channel which is only on the air for license-saver purposes; it would be an extra precaution against accidental usage. Key-ups on the license-saver site could interfere with transmissions on a nearby operational site; now they're stand-alone with some overlapping frequencies but eventually they'll be simulcast.
