Chicago UHF Med Channels

Does anyone have a list of the Chicago UHF med channels used for Ambulance to Hospital patient reports? 463Mhz looking for freqs and tones. I know they are spread out on the Hospital page but I thought there was a dedicated list somewhere.

I downloaded the statewide database as a csv and selected the 463-464 frequencies tagged as Hospital.

Many years ago, I put together a comprehensive list here and had them all listed, but was removed or someone thought it would be better to put them under the hospital and I let it be. They're not used as much as they used to be, and CFD is encouraged to use cell phones instead of radios for patient privacy.
