I found this on the UWIN Site,

Scroll down and open the 'Narrowband Migration Report (12/10/04)'. Then scroll down to appendix D, and there is a list of all current UHF/VHF frequencies used in the state, along with the counties that use them.
I copied and pasted to an Excel file, and then migrated that into a UASD file, and it's remarkably accurate.
Hope you all can make use of it! Email me if you want the UASD file for my 396.
