When I create a favorite list, I avoid all other sites in my state that could cause problems.
In doing so, I select permanent avoid -- but only when creating a specific favorite list.

This does not permanently avoid these other sites in other situations, does it?

If you mean does it avoid it in the main database or other favorites lists, the answer is no - the objects are treated separately in the scanner. Site xyz in system abc in a favorites list is a separate item from the site xyz in system abc that's in the main database.

When you create a Favorites list, why not remove the unwanted sites instead of avoiding them?
If you have a multi-site system in your state, you can create a favorite list for each site that you may be in with only the single site frequencies.

Just a thought!

Edit your favoties list by selecting the star with the pencil icon in Sentinel. Once you're in editing mode, you can delete and add info as you choose. As mentioned by Jkahn, you can go down the list of sites, deleting those sites that are over 50 miles from you (unless you frequent them often).
