first responder provides inital treatment to a patient typically basic life support measures, a " squad, medic or ambulance provides advanced treatment , or transportation for a patient, all depending upon the level of providers on the vehicle.
Lucas County EMS is a "tiered" EMS system: The fire departments provide initial on scene care and thus are called "first responders" although almost are trained to the EMT-Basic level. Some jurisdictions (such as Springfield) provide Advanced Life Support first response with at least one Paramedic on the fire apparatus.
The Life Squads provide ALS transport and are always staffed by 2 Paramedics.
Private ambulance companies such as MedCorp, Life Star, Rumpf, etc. provide BLS transport for Toledo and some other suburbs. Most suburban FDs send a transport unit as the "first responder" (examples are the Springfield and Sylvania "Medic" Units) and can transport BLS patients themselves.
