Well according to a list with a breakdown of all ALS Units and thier IDs:
2=Englewood Hospital ie 201=ALS Unit from Englewood Hospital
9=Administration
1=Unit Number
I dont see anything about a 4th digit. But it most likely is some type of administrative unit based out of Englewood Hospital.
