Has anyone been able to successfully program the MFD tone outs on a SDS 100 or 200? If so, can I get some guidance?

Can't speak specifically for your agency of interest but have a read at the easier to read manual excerpt below:
Yeah, I have read that. I guess I'm a little confused.

When in Tone-Out Standby mode the following confuses me.

1. It only scans one frequency. Is it possible to scan all of the tone out channels?
2. I can't hear any audio when the channel is transmitting (based on RSSI--I assume this is because it is a trunked frequency). How is it possible for me to identify who/where the tone out is intended for?

And in addition to that, do you have the A and B tones?
No, I do not have the AB codes. I have used the search mode and see them display, but beyond that, I can't identify what id should go with them because I can't hear the audio.

I do not believe the tone out feature on Unidens can handle trunked systems as there is nothing related to trunked in the tone out section.

I dont know specifically how Midland is set up, I'm assuming you're talking about the tones that are sent over the TG before the dispatch info.

If so, that's just audio to grab attention. Most fire stations are set up to where the dispatch console sends out a digital signal to the receiving radio that then opens relays that turns on lights and opens the speakers. The two tone is just audio only.

Op is trying to set up Unidens FTO which is meant for conventional freqs and it seems op's tone outs go over trunked system.

Looking at the Midland County database page, it shows that the fire dispatch is rebroadcasted on a conventional frequency. If the tone outs are included in what's carried on the rebroadcast frequency, tone out may work, But if only carried on the trunked system, as already noted, you'll be unable to catch them.
