Not necessarily. Down tilt is used mainly for two things.
1) On high level sites it's used to reduce the range to radio horizon. There are still some "Old Timers" who believe that a 1kW transmitter on a 4, 000 foot mountain top is the way to go. I digress.
2) to protect someone behind you. I'm working on a system right now that uses down tilt to protect a co-channel user behind the coverage area.
Downtilt is not mandated. It's used in system design to customize coverage.
