over the ocean air/ground comms question

I figure someone here would probably know the answer to this:

When an a/c is over the ocean or far enough from terrestial comms, what is used to contact the ground? Sat phone or similar?

For example, if theres a medical emergency on board (AA61, right this moment) how does the convenient onboard ER doctor talk to american's flight medical service to make decisions?

They use LDOC frequencies like this one,

They can use an HF LDOC phone patch but it's far more likely they would use SATCOM.

L-band satellite (INMARSAT) carries a form of ACARS. Some of decode that. There is a lot more of that than HF.

If you can get a good signal on
(INMARSAT) you can decode it. I have the antenna to do it but i live in a apartment building and my window is facing the wrong direction to receive the signals so I am out of luck.

I get a fine signal with several different antennas including some not very large. (I live in northern Virginia.) It does take an antenna designed for that range (some under $100 and small enough that they are easily portable) and an LNA. Usually I use an Airspy R2 with it. I get the best reception with a 4 ft L-band dish on a tripod in my bedroom (my house has "thin" walls).

This is what I do. Some of those medical calls are interesting too
