1) The use of some form of repeater access control is mandated by FCC regs.

2) The selection of the access control system type, and specific values (such as PL tones or DPL codes) is up to the system licensee, as far as the FCC is concerned.

3) In some states, regions or districts, specific values are worked out as part of a state-, region- or district-wide communications plan.

The only system I have seen is where it is worked out between the licensee and the frequency coordinator.

Often there is no coordinator and the licensee figures it out for himself.

A frequency coordination has nothing to do with PL/DPL assignments.

I'm not where I have the books right now; it is in the Tech Spec section of Part 90 (under "non-voice" as I seem to recall).
I looked through Part 90 and although there is plenty of comments about non-voice use and protection against unauthorized access to transmitters, I can find nothing that dictates the requirement to use CTCSS, DCS, or NAC on any Part 90 system. Since there are lots of public safety repeaters in use around the nation that have no tone or code access required to access, I cannot believe that all those different radio shops would be ignoring a requirement.

I think your confusing the fact that you need a way to actually control the repeater/controller such as DTMF or Phone line.

Or maybe an IB requirement with a PW requirement (or some other part entirely).

I have done two Part 90 coordinations over the years and both involved the state APCO representative. He asked on both pairs of frequencies what "tone" I was planning to use. I had done some monitoring and determined what not to use. The FCC form doesn't have a place for tone access, but the coordinators must have something in their database to keep nearby systems from interfering with one another.
