At a minimum, it is just coding the digitial signal enough
that a typical user can't make sense of the conversation.

On the other hand, if the communication is really sensitive,
the encryption could be strong enough that even if you
tried you couldn't "break into it".

Yes,

Encryption is scrambling, and included all you mentioned.

There are analog methods like inverion, and digital methods like DVP, DES, AES, etc.
