The Motorola XPR7550 UHF antenna is about 3.5 Inches. Are they part of a larger group or owned by a holding company using a different name? Locating the frequency via the FCC database can be a challenge if the license isn't under Capital Manor.

Doing a site search in FCC database shows nothing within 1 mile of the facility other than Polk County, City of Salem, Salem Electric, and one other unrelated license. My bet is they are using unlicensed freqs or rental radios.

If you are near by try close call/spectrum sweeper.

RaleighGuy is probably spot on. My local hospital ER Security and County Fairground use GMRS and FRS handhelds. I've listened to some nursing home traffic, when you find it you're going to be disappointed I fear as it's mostly bed wetting and ground level falls.

I'm not familiar with the name, but if it's like most senior facilities (like The Springs), the rooms have automated call buttons that will announce over the radio the room number and what priority was pushed. Make sure your search includes the "input" freqs 455-460 and 465-470. Around here they were still using FM, but newer places might be on DMR. I found 3 facilities in my area just by accident. A search of the ULS will show nothing. The companies sell these places all the cool tech, but don't bother to tell them it can be heard miles away and that they need a license.
