I think we have progress with _1120. But still only hearing audio from LCN1 (451.700 slot 1).

See log snip below, but...
The voice calls you see for 218 and 230 in the P25RX log did not really occur on frequency 451.700 (Ch1/Ch2); they actually occurred on 461.675 (Ch3/Ch4).

See comparison of logs below:

I don't get why DSD is showing LCN 3,4 when the activity are for the LCN 1,2. Can you please run 10-11_1421 and some of the logs? I see there are some extra bits associated with activity. This version will show them. It might help to understand.

I will run _1421, but things are starting to slow down here in Boston so might have to wait for workday to start tomorrow for sufficient data.
The system has 3 frequencies:
Frequency
451.7000 Ch 01 / 02
461.6750 Ch.03 / 04
463.5500 Ch.05 / 06

Those calls on TG 218 and 230 in my previous post were on 461.6750.

Circling back to let everyone following this thread know that Cap+ voice following is now working. Huge thanks to @btt .

I've tested on several different New England systems over the past couple days. The P25RX follows the rest channel and follows voice transmissions. Talk groups are entered correctly into the TG editor. I've been running it side by side with a 2-dongle DSD+FL setup and find the experience comparable. The only difference currently noted is that voice call priority is not working for Cap+ in the same fashion as for P25 trunked systems (e.g. no mid-call interrupt).

To get this working, on the DMR tab enter all channel frequencies, and check the Control Channel box appropriately (in all the Cap+ systems I follow, every frequency has potential to be a rest channel=control channel). Each frequency needs to be entered only once (that is, you don't need to enter the same frequency twice to account for both slots).
Make sure the Con+ Control radio button is clicked below and write the configuration. It may take a few minutes to fully optimize.
