How do I hear audio? As I have my audio output set to the VB Cable -- no audio can be heard from the default sound card/speakers.
You are close, but your audio input and output are not set correctly based on your screen capture in your first post.
In the lower right corner of your capture there are three blocks/paragraphs about audio devices. The first (input) is three lines of what DSD+ found. The second (output) two lines of what DSD+ found. The third is how DSD+ connected by number to your inputs/outputs without being directed by command line switches when you started DSD+.
Now that you know the numbers you need to close DSD+ and restart with command line switches to make the correct connection.
Output from your SDR program must be directed to a VAC cable. That cable is to become the input to DSD+. DSD+ must be connected to the speakers.
Once DSD+ has input from the SDR then the pane in the upper right will no longer be flat lined and should show some ripple noise. When receiving a signal you should see higher amplitude signal looking a little like distorted sine waves. At this point you should be able to decode control channel information which is easier than voice channel for me.
Decoding voice is not like turning on a radio and dialing in the frequency. You are going to have to play with frequency, bandwidth and amplitude. Once you are at that point then look into the fine tuning parameter to improve the decode. Also brush up on your bat file skills to make it easier to start DSD+ with the command switches without a lot of keyboard typing.
