I see the DB was updated yesterday and now list the Sayreville 700TRS as a simulcast for the Middlesex county system, is that a full simulcast for the entire system or just added coverage for the towns that use the county system in that area

The systems were actually merged on Sunday. There is the County Simulcast and there is the Sayreville Simulcast, exactly as listed. One covers the entire county, the other covers the borough. Both cells are on the same core, and as such are technically one system, even though there may be some restrictions in place as far as what talkgroups and/or radios can roam between cells.

Bottom line: it's functionally no different than when the two were listed as separate systems

Ok, monitoring both systems in open scan it seems nothing but Sayreville is coming over on that system and they are also coming through on the county system, like u said exactly as when they were two separate systems. Thank you for the info

If you are in or nearer to Sayreville than any of the sites for the County's cell, I assume that the scanner is going to favor the borough's cell. It all depends on which control channel it finds first. I have no idea what scanner or other type of receiver you're using, but certainly you could program it to only scan one site or the other if necessary.

That’s what I was messing around with, but the Sayreville site is not carrying any other traffic than it’s own, unless like u said a sub radio enters that area and switches then I would think that traffic would be carried

Given the fact that the Sayreville cell only has four frequencies (6 voice paths with TDMA), it's far more likely that you'd hear Sayreville traffic roaming to the county cell vs county traffic on the Sayreville cell. It's fairly simple to restrict talkgroup roaming per site at the system level, but I don't have any insight as to how they've set that system up. I recall a thread from a year or two ago with some discussion of roaming between these two cells.
