Motorola Programming Template For Federal Users

Does anybody here recall running across a template, possibly from the USFS, that had columns of data for programming radios like the Motorola XTS5000? I seem to recall running across that in the past, but I can't seem to locate it now. It was handy because it had the programming information in the same order as you would use for programming an XTS5000, etc. The columns were marked TX FREQ, TX PL, RX FREQ, RX PL, or something along those line.

I recall the template your talking about, I believe I saw it on the dhs website where you download the national interoperability field guide but don't remember for sure. I'd check but I'm on my mobile at the moment. Perhaps someone else will chime in with a more precise location.
