416 would be the input, so the Repeater is 407.8375

Are you hearing all of the transmission or part of them ?
if Part, then look for the Control channel for a TRS

I'm guessing the VA has finally turned on a UHF Trunk

I am hearing both sides same nac and I am in small town Springfield mo I would not think the va would set up a whole trunk just for one facility location these are the only 2 frequencies in the federal uhf band that show this nac

A few of the recent FBO.gov were for TRS systems, which was my original thought.

Checking the VA Web site, there are 3 possibilities for Springfield MO

Gene Taylor Veterans' Outpatient Clinic - Veterans Health Care System of the Ozarks
altho most Outpatient Clinics I've seen are simplex, if more than one Officer on-duty, otherwise it is Phone

Springfield, MO Vet Center - Locations
VET Centers, generally get a visit from time-time from the area VA Police, but nothing much for Full-time RF

Springfield National Cemetery - National Cemetery Administration
Here in Mass out National Cemetary went DMR

Then again, I am only going with VA based on 95% of my entries for that Repeater pair are VA

It might not be trunked. Search 406-420 in 12.5 kHz steps and see if you hear a control channel.

We have a new va facility on the south end of town, the only other one is at least 20miles in mount Vernon mo I am sure would not be hearing them on uhf, and I search the federal band 24 7 and no indication of a control channel

UHF can and does carry, 20 mi is nothing, depending on topography/terrain

You might try a road trip to the VA facility, sit in the parking lot and when they talk take the antenna off, see if you still hear them
