It can be a number of issues, more than likely reception.
Also, cell phone sites can plague these sensitive scanners, due to the high RF saturation in the area (in frequency ranges close to these trunking sites).
Multipath isn't much of an issue, and less for P25, but downtown I'd imagine it could still be a problem.
I use an RS 800 MHz antenna, but I'm not sure if they make it anymore. They also had a "stubby" antenna for racing fans (I think), that, due to it's size, did well in high power saturation areas (close to the sites, where the scanner gets overloaded; I think this was a no-gain antenna).
One way to know if you're close to the site, and overloading (power) might be the problem, is to remove the antenna to see if the reception gets better. I ran an audio feed antenna-less for awhile (I was < 100 yds from the transmitter).
