The other one in the dbase - 2641, I have heard a few times as they refer to it as station 8, and maention Gladwin where as the one above said station 1.
At the pdf link if you count the list under medical offices and continuing care the midland one is number 8 in the list, granted its not exact evidence but something to go on hehe.
