I often scan the HEAR (155.340) in my mobile, base stations and often hear hi-lo tones but I don’t seem to hear them often near Huntsville where I thought they might be originating from the hospital. Any ideas?

They are coming out of Tennessee. I don't remember where anymore, though.
Must be a solid transmitter, they come in loud and clear even on a mobile in the Quad Cities and Lawrence Co area! Don’t hear the accompanying voice very often unless I’m on a base

These come from an entity called Jackson Med-Link in Jackson, TN. I assume they are similar to Alabama's TCC. I only ever hear a 3-digit DTMF string followed by the hi-lo alert tone, and sometimes a hospital replying "go ahead". Given this, I suspect a ground ambulance will request Jackson Med-Link to 'Ring down" a specific hospital, at which point they send the DTMF string to open up the specific hospital's HEAR radio, then use the hi-lo tone to get their attention. The ground ambulance can then give their patient report to the hospital. This is all assumption, based on what I can hear and how other areas have used the HEAR channel in the past.

You are correct Jackson Med link does what you say for all ground units and all air units (Medical Helicopters) Med-Link keeps up with inbound numbers to help with Hospital Availability.
