You are extending your reception range from those departments that serve areas within 10 miles of your location, to departments that serve areas within 20 miles of your location. At least that's the ideal case. Sometimes the defined service area of a department is much larger than it should be, so you may pick up departments that serve areas in excess of 20 miles of your location.

One word edit to Voyager's explanation: change "...extending your reception range..." to "...extending the selection range...". It doesn't actually affect reception (i.e. sensitivity) but rather what gets selected for scanning.

One thing I am confused about. I have a repeater set to 15miles and my location set to 0. I change the repeater to 10 miles so I don't pick it up any more when I'm set to 0 but when I change my range to 10 or even max at 50 it doesn't turn the repeater on?

If you are accurately describing what you are doing, the repeater would be enabled for scanning.

That is what my thought is too. If I increase the range back to 15 it works. And this is me stationary.

I haven't tried it mobile with GPS. I'll report back once I've done more testing.
