Ten Tec 1340 Q15 Transistor Blew

Installed a ten tec 1340 today, (well, over the course of a week or so) and hooked it to power, and attached a fused 12v source only to have Q15 (2SC2166) Transistor blow it's right side lead. Kinda declined when it comes to electronics, I'm kinda curious if anyone could help me. I'm not sure if the part malfunctioned, or if it was something i have done. The + and - leads to the key are apart and show zero continuity (without power in). So i'm not sure what this issue is.

The manual states to make sure theres no contact between the back of the transistor to the aluminum heatsink. No contact noted

After a quick call (during store hours) with Ten Tec, I will recheck my 12v source. Seems as it may be that my 12 v source has been connected backwards!
