Multiple Errors While Using DSD+

This is my first time doing this. There is a local hospital that uses a DMR repeater & I would like to find the group codes used. I'm currently using a PRO-668 scanner with the discriminator output. I am getting the sound a radio makes when it is keyed up but no voice. I am getting a bunch of errors as pictured below in a screenshot. Can anyone explain what I am doing wrong here or is this what its supposed to look like.

Can you record and post an audio sample? That's by far the easiest way to troubleshoot issues like this. Could be any number of things... weak signal, bad audio levels etc etc.

Dumb question, how do I upload audio?

Press R is DSD+ to start and stop raw audio recording. 30 seconds is a good length. Upload to zippyshare.com and post the link here.
